Internship Application Guidelines

Application Period

• Summer internship applications are now open and will continue until April 30, 2021.

• Limited slots are available per department, allocated on a first-come, first-served basis.

• No applications will be accepted after quotas are filled.

Mandatory Requirements for All Interns

1. Adherence to Rules:

   • Compliance with hospital working hours, dress code, and professional conduct.

2. Orientation:

   • Mandatory hospital orientation training at the start of the internship.

Application Documents

Submit the following to the Hospital Training Unit:

1. Internship Application Form (2 copies)

2. Vocational Training Contract (3 copies)

   • Must be prepared by the school per Vocational Training Law No. 3308.

3. Photocopy of ID Card (1 copy)

4. Passport-Sized Photo (1 copy)

Documents to Bring on the First Day

1. Social Security Employment Declaration (Sigortalı İşe Giriş Bildirgesi)

   • Must list: Profession Code 9901.02 ("Trainee Student").

   • Obtain from your school or the Social Security Institution (SGK).

   • No internship will begin without this document.

2. Internship Logbook (Staj Uygulama Defteri)

   • Must include:

     • Student Evaluation Form

     • Attendance Sheet

   • Not required if the school does not use logbooks (the hospital will provide an attendance sheet).

3. Occupational Health & Safety Training Certificate (if available)

4. Student ID Card

   • Must be worn visibly throughout the internship.
