Patient Responsibilities  

1. **Referral Compliance:**  
   - Follow the referral chain established by the **Ministry of Health** and other **social security institutions**.  

2. **Update Personal Information:**  
   - Promptly notify the hospital of any changes in your **health, social security, or personal details**.  

3. **Appointment Adherence:**  
   - Arrive **on time** for scheduled appointments. Cancel or reschedule in advance if necessary.  

4. **Accurate Medical History:**  
   - Provide **complete and truthful information** about:  
     - Current symptoms  
     - Past illnesses/hospitalizations  
     - Medications you are taking  

5. **Cooperation with Healthcare Staff:**  

   - Actively participate in your **treatment, care, and rehabilitation** process.  

6. **Respect for Others:**  
   - Be considerate of **hospital staff, other patients, and visitors**.  

7. **Compensation for Damages:**  
   - Cover costs for any **intentional damage** to hospital property.  

8. **Understand Treatment Plans:**  
   - Listen carefully to **medical instructions** and ask questions if unclear.  

9. **Post-Discharge Compliance:**  
   - Confirm you fully understand **home care instructions, medications, and follow-up requirements** before leaving.  

10. **Honesty About Non-Compliance:**  
    - Inform healthcare providers if you **cannot follow** prescribed treatments.  

11. **Self-Care for Minor Needs:**  
    - Manage **basic personal care** when medically appropriate.  

12. **Consequences of Refusal:**
