First-time applications for Medical Board procedures (e.g., disability reports, ÇÖZGER reports, firearm license/status reports) require an appointment via the 182 MHRS (Central Physician Appointment System).
Meeting Schedule:
Our Medical Board convenes once a week (Wednesdays only).
Document Verification:
Verify all details on your examination form (ID, address, phone number, photo, etc.) for accuracy.
Ensure the documents belong to you.
Mandatory Attendance:
The person named in the report must attend the Medical Board in person at the scheduled date/time with a valid ID.
No exceptions: If the individual or their ID is absent, the process will not begin.
Minors (Under 18):
A parent or legal guardian must accompany the minor throughout the Medical Board process.
Submission of Documents:
After examinations, submit all required documents (test results, scans, medication reports, epicrisis reports, etc.) to the Medical Board Unit.
Make copies in advance if you need to retain any documents. No documents will be returned after submission.
Delayed Tests:
If requested tests will take longer than expected, notify the Medical Board Unit immediately.
Avoid Unauthorized Assistance:
Do not engage with individuals offering unauthorized help.
Report any misconduct to hospital authorities.
